Select the form that matches the path you confirmed in Step One. You can complete it entirely online, or download a PDF and return it by email. Submission does not constitute acceptance; it begins the review process.
Captures your accreditation basis, investment experience, source of funds, and the documentation needed for third party verification under Rule 506(c).
Captures your non U.S. status, residency, and the representations required under Reg S Category 3, including the distribution compliance period and resale restrictions.
Once your questionnaire is submitted, an independent third party verifier reviews your eligibility. This is a requirement of Rule 506(c) for U.S. investors and standard diligence for Reg S investors. You stay in the loop the entire time.
A CPA letter, a brokerage or bank statement, a recent W-2 or tax return, or a confirmation letter from your attorney. We work with you to choose the lightest option.
An accredited investor verification service (or qualified attorney or CPA letter) issues the formal verification. The Company does not handle your sensitive financials directly.
On approval, you receive an email confirming your verified status, along with unique credentials for the secure document portal used in Step Four.
Once verified, you sign in below with the credentials we sent you. Take as long as you need. We strongly encourage you to share these documents with your attorney, CPA, or financial advisor before making any decision.

If you decide to invest, you complete three things. Each one is structured to be straightforward.
You sign electronically through our document platform. The Subscription Agreement specifies your investment amount, share count at $0.10 per share, and your representations as an investor.
Wire funds to the account named in the funding instructions you receive after signing. Funds are held pending acceptance by the Company.
The Company countersigns your Subscription Agreement, your shares are recorded on the cap table, and you receive a written confirmation. You are now a Doradus shareholder.
Confidential materials are available only to authorized users. Submission of an intake form or investor questionnaire does not guarantee approval, qualification, or acceptance as an investor in Doradus Quantum Labs, Inc.
All offerings are made solely under, and qualified entirely by, the Company's Private Placement Memorandum (PPM). Securities offered are not registered with the U.S. Securities and Exchange Commission and are sold only to accredited investors under Regulation D Rule 506(c) or to eligible non U.S. persons under Regulation S Category 3.
